How hard and is it practical to file bankruptcy myself? What is the average cost without a lawyer?
Do they have inexpensive forms that I can obtain?

Try the United States Bankruptcy Clerk's Office for forms. They may have fill in the blank forms for conusumer use. Filing on your own without at least a consultation is risky. If you get into trouble, the court will protect you somewhat but you will be charged with deadlines and certain knowledge required of all debtors and their counsel.
