Inadequate medical treatment

I was the driver of a car that was hit full force on the passenger door/wheel well area. My doctor referred me to an orthopedic surgeon for a left shoulder injury. The surgeon said I had some pulled muscles, sent me for physical therapy, threw drugs at me and told me I was fine. He never spent more than 5 minutes per visit, kept me coming back once a month for a year then told me I was fine and to stop being a "whimp."

I went to another doctor because I was getting worse, not better. The shoulder injury now involves my hand, wrist, elbow, shoulder, neck, back and hip. Muscles spasms like you wouldn't believe. I haven't been able to work since the accident (18 months ago) and still can't work. (I'm self-employed - computer graphics).

Do I have any kind of case for inadequate medical treatment?

It appears that your injury was caused by the auto accident. While the first doctor may have treated you improperly, it is not clear from your posting that the first doctor caused any harm. Without harm, you can't sue the doctor. What you should consider is a lawsuit against the two automobile operators.
