Legal Question in Personal Injury in Indiana

5 yr old broke elbow at YMCA no report done

there were 2 teachers outside w/ 6 children a 5 yr old was climbing the ladder and fell. He broke his elbow. One of the teachers picked him up, carried him inside to his mother who works there instead of calling 911 like she was trained to do and never filled out an incident report. When asked where the child fell from she can only say ''I think he was right here, but I know he landed here, cause thats where I picked him up at''. Which means between two adults noone was watching the little boy.
