I will soon be a grandmother. My daughter is 17 and she will give birth to a baby boy in January 2004. She will be changing her last name to my maiden name when she turns 18, in November 2004. Can she give the baby my maiden name on the birth certificate when the baby is born so that they will have the same last name in November?

Yes. In Pennsylvania, the parents can give the baby any last name they wish. It's certainly easier to put it on the birth certificate to start, rather than change it later.
