Step-parent adoption

My husband was given sole legal and physical custody, his ex was given supervised vistitation due to her mental illness. Do we still need her consent for me to adopt the kids? If so, is there a way that we can get her rights removed so that I can adopt the kids?

Yes, you either need the mother's consent to the adoption or, in the alternative, her parental rights must be terminated by a court of proper jurisdiction as part of the process of adjudicating your petition for adoption.
