requesting a form

I am trying to find a form for a motion to reopen a case. it is in a child visitation case. I have to submit this myself cause I represent myself

There really isn't such a form. I suggest you get a plain piece of paper, put the caption at the top, they put "Motion to Reopen", and then as clearly as possible tell the court why you want to reopen the case and what you will be asking to do if the court grants your motion. At the bottom put you name, address and phone number and sign above your name. Take that to the clerk of court's office and they will tell you how much you have to pay to file your motion.
